Gravecrawler + Phyrexian Altar + Plague Belcher
once all 3 permanents are on the battlefield, sacrifice the gravecrawler to the phyrexian altar to add one black mana to your mana pool. The gravecrawler dying will trigger the plague belcher's second ability and each opponent will lose one life. You will then use the one black mana in your mana pool to cast the gravecrawler from the graveyard. You will then start the combo over and repeat until someone is able to stop it, or you have won the game.
